Monday, Tuesday, Wednesday:  8am – 4pm, Thursday:  8am – 6pm, Friday:  8am – 1pm

• WIC staff will determine if you are income eligible for services.

• WIC staff will obtain your height, weight, and blood iron measurements.

• A WIC Health Professional will provide nutrition education and determine a nutritional or medical risk.

If eligible, you will receive coupons to buy nutritious foods at a local WIC authorized grocery store.

• Nutrition Education and  support

• Breastfeeding education and support.

• Referral for health care.

• Immunization screening and referral.

• Supplemental foods like:

  • Milk, Cereal, Juices, Eggs, Peanut Butter, Beans, Whole Grains, Fruits and Vegetables

  • Plus Tuna Fish and Carrots if you are breastfeeding

  • Infant Formula and Baby Foods

You must have a gross income below these guidelines (before taxes or payroll deductions):

• Proof of income (current pay stubs, approval letter for Healthy Start, Ohio Works First, Food Stamps, or current Medicaid card)

• Proof of address (utility or credit bill, or Ohio Driver’s License)

• Proof of identity for you and any other applicants (birth certificate, driver’s license, Medicaid Card, crib card, shot record)

• All family members applying for WIC services

• If pregnant, a doctor’s statement showing due date

• Children’s shot records
